Yes, but you can complete all forms online before your visit. We'll send you a link when you book. If you prefer, you can fill them out in our lobby — just arrive 15 minutes early.
The $99 special is for patients without insurance. If you have coverage, we'll bill your insurance directly and you'll only pay your normal copay, which is often even less.
